• Physician’s Statement 
• Certified True Copy of Death Certificate 
• Certified True Copy of deceased’s IC as evidence of age if proof has not been received before 
• Post Mortem Report (where available) 
• Certified True Copy of proof of relationship (e.g.: Marriage Certificate/Birth Certificate) 
• Certified True Copy of claimant’s IC 
• Certified True Copy of Police Report (for Accident Death only) 
• Consent Letter for medical report extraction (4 copies) 
• Certified True Copy of Burial Certificate 
• Certified True Copy “Sijil Faraid / Letter of Administration (if applicable) 
• Newspaper cutting (if any) 
• Other supporting documents (if applicable)
